Veterans' Assistance
About the Program
The purpose of the Veterans Assistance Program is to provide needed financial grants to fellow Patriots, Veterans and their families at a time of need.
We understand how difficult it can be to ask others for help but there are times when we all need a hand up during a difficult time. The Department of Florida provides small grants to all Veterans and their families residing in Florida in an expedient and respectful manner.
In order to process your assistance request we will need the following documentation:

Copy of the Veteran’s Member #4 DD214 (black out Social Security Number prior to sending)

Copy of Individual’s photo ID

The invoice or bill requiring assistance

Filled out Department of Florida Veterans Assistance Program Application

Download and fill out the Application form, mail it to Department of Florida, 1325 Grasslands Blvd., Lakeland, FL 33803.
